a. Start with a File 6 x 2 in with 72 dpi

b. Make a new Layer by hitting the New Layer Button

c. Name the layer Base by double clicking on it.

d. Hit D to reset your colors to Black and White.

e. Go to Filter>Render>Clouds

f. Go to Image>Adjustments>Brightness/Contrast...

and adjust it to these settings:

Contrast: -45

a. Switch over to the Channels Pallete by clicking on the Channels Tab in the Layers Pallete.

b. Make a new channel by clicking on the New Channel button:

c. Go to Filter>Render>Difference Clouds

d. Now hit Ctrl + F a couple times until you have something similar to the image to the right.

e. Finally go to Filter>Stylize>Find Edges

a. Now switch back to the layers pallete and click on the Base layer.

b. Go to Filter>Render>Lighting Effects...

use these settings:

Light Type: Spotlight

On: Checked | Intensity: 35

Focus: 69 | Gloss: 69

Exposure: 0 | Ambience: 8

Texture Channel: Alpha 1

White is high: Unchecked | Height: 10

a. Adjust the Brightness/Contrast...

using these settings:

Contrast: -85

b. Next go to Filter>Noise>Add Noise...

use these settings:

Amount: 2% | Distribution: Gaussian

Monochromatic: Checked

c. Add Texture on Page 2
